For these individuals, the calculus shifts. The modest risks associated with Sitagliptin—which is generally well-tolerated and carries a low intrinsic risk of hypoglycemia—may be outweighed by the potential benefit of slowing beta cell decline before it becomes irreversible.
What Patients Should Know Before Accepting—or Refusing—the Prescription
If your physician has suggested Sitagliptin and you are uncertain about the recommendation, the most productive response is not to dismiss it but to engage with it. Ask specifically why the medication is being proposed now, what the expected benefit is over a defined timeframe, and what the monitoring plan looks like going forward.
It is equally reasonable to ask about alternatives. In some cases, metformin remains the preferred first-line agent, particularly given its long safety record, low cost, and additional data on cardiovascular risk reduction. Sitagliptin may be recommended instead of or alongside metformin depending on kidney function, gastrointestinal tolerability, and individual clinical goals.
One thing worth understanding clearly: accepting a prescription for Sitagliptin at an early stage is not a concession that your condition is worse than you thought. In many cases, it reflects a physician's intent to keep it from becoming so.

American medicine has historically been reactive in its approach to chronic disease—treating problems after they materialize rather than investing in prevention once risk is established. Diabetes care is, slowly, moving in a different direction.
The emergence of medications with demonstrated protective properties, combined with better tools for identifying high-risk patients earlier, has created genuine opportunities to intervene before the window for meaningful prevention closes. Sitagliptin is one instrument in that evolving toolkit.
For patients who find themselves holding a prescription they did not expect, the surprise may ultimately be a useful one. A medication recommended before your numbers look serious is, in many respects, a medication recommended at exactly the right time.
