For patients who have read about semaglutide's weight loss outcomes or empagliflozin's heart failure benefits, this can be puzzling. Why would a physician recommend sitagliptin when newer options appear, on the surface, to offer more? The answer lies in a set of clinical and practical considerations that rarely make it into popular health coverage—but that matter enormously in the exam room.

The Trial Data Versus the Real-World Patient

Clinical trials are designed to demonstrate what a drug can do under controlled conditions. Participants are typically selected to minimize confounding variables, which means the populations studied in landmark GLP-1 and SGLT-2 trials were not always representative of the broader American patient population. Many pivotal cardiovascular outcome trials, for instance, enrolled patients who already had established cardiovascular disease—a meaningful distinction when a physician is treating someone who does not yet fall into that high-risk category.

Sitagliptin's own cardiovascular outcome trial, TECOS, demonstrated that it neither increased nor decreased major cardiovascular events, establishing a robust safety record across a broad patient population. For a physician managing a patient with well-controlled blood sugar, no established heart disease, and a primary goal of maintaining glycemic stability without adding complexity, that neutral cardiovascular profile is not a liability—it is a feature.

Tolerability: The Underrated Factor in Long-Term Adherence

Medication adherence is one of the most persistent challenges in diabetes management. A drug that produces significant results in a clinical trial but causes intolerable side effects in daily life offers limited real-world value. This is where sitagliptin holds a meaningful, if understated, advantage.

GLP-1 receptor agonists, particularly the injectable formulations, are associated with nausea, vomiting, and gastrointestinal discomfort that can be severe enough to prompt discontinuation. Even oral GLP-1 agents carry similar gastrointestinal profiles for a substantial proportion of users. SGLT-2 inhibitors carry risks of urinary tract infections, genital mycotic infections, and, in certain patients, a rare but serious condition called diabetic ketoacidosis.

Sitagliptin, by contrast, is generally well tolerated. It does not cause hypoglycemia when used as monotherapy, produces minimal gastrointestinal effects, and is taken once daily in oral tablet form without the titration schedules required by many newer agents. For patients who have already struggled with medication side effects, or who have comorbidities that increase the risk profile of alternative therapies, this tolerability record is clinically significant.

Cost, Coverage, and the American Healthcare Reality

No discussion of prescribing decisions in the United States is complete without addressing cost. The list prices of newer diabetes medications remain high, and while manufacturer assistance programs and insurance coverage can reduce out-of-pocket expenses for some patients, access is far from universal. Prior authorization requirements, formulary restrictions, and coverage gaps create real barriers that affect whether a patient actually takes the medication prescribed.

Sitagliptin is available as a generic in the United States, and generic versions have significantly reduced the cost burden for many patients. For individuals on Medicare, fixed-income households, or plans with limited specialty drug coverage, the difference in annual medication cost between sitagliptin and a branded GLP-1 agent can be substantial—sometimes thousands of dollars. A medication that a patient can consistently afford and access will, in most cases, outperform a theoretically superior drug that sits uncollected at the pharmacy counter.

Individual Patient Profiles and the Limits of Generalization

Clinical guidelines from organizations such as the American Diabetes Association have increasingly emphasized individualized treatment decisions, moving away from rigid step-therapy algorithms toward a framework that considers cardiovascular risk, kidney function, weight goals, hypoglycemia risk, patient preferences, and cost. Within this framework, sitagliptin fits a meaningful niche.

Consider a patient in their mid-fifties with well-managed type 2 diabetes, no established cardiovascular disease, normal kidney function, a stable weight, and a history of difficulty tolerating gastrointestinal side effects from prior medications. For this individual, the incremental benefit of a GLP-1 agent's weight loss effect may be modest, while the risk of side effects that disrupt adherence is real. Sitagliptin's mechanism—enhancing the body's own incretin response to lower blood sugar in a glucose-dependent manner—aligns well with the goal of maintaining glycemic control without overcorrecting.

Similarly, older adults who are at elevated risk of falls and for whom hypoglycemia carries serious consequences may benefit from sitagliptin's low hypoglycemia profile. Patients with certain gastrointestinal conditions, those who are uncomfortable with injectable medications, or individuals managing multiple chronic conditions with complex medication regimens may all represent cases where sitagliptin's predictability and simplicity are genuine clinical advantages.

What 'Better' Actually Means in Clinical Context

The framing of newer medications as simply 'better' reflects a misunderstanding of how therapeutic value is assessed in practice. Better for whom? Better under what circumstances? Better relative to which alternative?

A GLP-1 receptor agonist may be the superior choice for a patient with obesity, elevated cardiovascular risk, and strong insurance coverage. An SGLT-2 inhibitor may be preferred for someone with heart failure or chronic kidney disease. But neither of these profiles describes every patient with type 2 diabetes, and the existence of impressive options for high-risk populations does not diminish the value of a well-tolerated, cost-accessible, evidence-supported medication for patients whose needs are different.

Physicians who continue to prescribe sitagliptin are not ignoring the evidence base for newer therapies. In most cases, they are applying that evidence carefully—recognizing that the patients in their practices do not always match the populations in landmark trials, and that the best medication is the one most likely to be taken consistently, tolerated well, and sustained over time.

A Measured Perspective for Informed Patients

For patients navigating these conversations with their care teams, the most productive approach is not to arrive with a predetermined conclusion about which drug is superior, but to understand the factors that inform the recommendation being made. Ask about cardiovascular risk stratification, discuss side effect tolerability, explore what your insurance will actually cover, and consider how a new medication fits into the full context of your health.

Sitagliptin's continued relevance in an era of therapeutic innovation is not a failure of medical progress—it is a reflection of the complexity of individualized care. In diabetes management, as in most areas of medicine, the right answer depends on the right question, and the right question is always: what is the best option for this specific patient, right now?
